Our local news station has been covering the Montana Life's End Institute all week on the evening news. They offer forms and information for your personal choice end of life. They also scan completed forms into their "bank" that is available to nationwide access so that wherever that person is, their medical provider and/or family can see their signed end of life directive and honor it.
I do not believe their services are free, I believe they are fee based, but if it is something you are interested in for yourself or your family, perhaps this site could be a start for you to find something similar in your own state.
