There is a cake that you can choose to purchase. WIth or without the cake, they put "pixie dust" and streamers on the table, sing a special song, and present a card signed by all the characters who are at the restaurant.

Pixie Dust is Mickey shaped sequin stuff.
 
Every time we go to LTT there seems to be everyone but us celebrating a birthday. One time we were given a table with glitter and streamers. I commented to the CM that we don't celebrate at WDW. She commented back that I must be joking as it seems there is no one that doesn't celebrate here and then she rolled her eyes! LOL! You get a few streamers and glitter. I saw many have large cakes that were ordered. Your best bet, since it is a special day, is to call ahead and order a cake. Of course you have to pay for it but a birthday is worth it!
 
I celebrated my birthday there 4 years ago (although I didn't know DH snuck up to the check-in counter to notify them!) It was exactly as kcashner described. I also received a huge chocolate cupcake for dessert - I think we got the regular dessert first, and then they brought this over to the table. I could be wrong, though - like I said, it was four years ago. The memory started going when I hit 21! :rotfl2:

It was really a lot of fun, but not too much to completely embarrass me! :blush: (And, I must admit, I loved the extra attention I got from my Pluto!)
 

We put pixie dust on your table and streamers then we sing happy birthday to you with a cupcake! I used to work there and the characters come over if they are near- but you get a group of the servers and host that sing and clap! I was there when they just had Minnie, Pluto, Chip, Dale and Meeko! I had a blast!
 
You should get a priority seating right at 4:00 so you can ring the bell as opening for dinner with the charcters you can request to ring it. It's especially special when it's your birthday!
 
Well, this explains why, when I made the ADR at LTT for October, and mentioned that it would be for my DAunt's 80th birthday dinner, she asked if she could note that down. She mentioned we can buy a cake, but there would be a cupcake!

I'll pass this thread along to the others in my party, maybe a cake would be a nice addition.

When we walked in at LTT and I saw the table I thought it was so nice. The streamers on it just made it to me. My boys are 7 years apart but 4 days apart so one year I thought I made a one day meal reservation. Disney had it on every day on every meal. On the last day my married oldest son, stood up in his chair and said," Hey, it's my birthday!" He was not happy with all the attention. We all thought it was great and I didn't even know a cake could be bought, lol.
