Has anyone else noticed that on the 2008 DDP, kids still get an appetizer?

Probably a typo. That'll be covered by "Subject to change without notice."

Or, it could be a result of changing most of the kids' meals to complete meals for one price. Example from 50's PTC:

Kids' Complete Meals

Grilled Fish of the Day served with whole-grain pilaf, veggies, and yogurt parfait $7.49

Grilled Chicken Strips with macaroni topped with marinara sauce and mozzarella, served with veggies and fruit salad $7.49

Choice of 1 appetizer, 1 entree and 1 dessert - $7.49

Appetizers: Chicken Noodle Soup OR Garden Salad

Entrees: Meatloaf OR Chicken Nuggets OR Macaroni and Cheese OR Hot Dog

Since the meals are all-inclusive for one price, they'd have to pretty much revamp the kids menus (again) to do away with the appetizer. I can't really see them doing that again this soon, or removing a part of the complete meal for DDP guests, but not OOP guests. Stranger things have happened, though, so we'll wait and see.

Besides, the kids' appetizer, with few exceptions, is not much of an appetizer. It's usually what you see above--the standard chicken noodle soup or garden salad. A few places allow their signature soups (Rose and Crown's potato leek and Le Cellier's cheddar cheese come to mind), but usually it's the generic chicken soup or salad.

Probably because the only way the kids' menus meet Disney's boasts about including all the food groups is by including the appetizer. I can't say that I'm surprised - whether it is about turning tables faster or saving money, those carrot sticks & ranch dressing don't have much impact! LOL

I am guessing it is because all those cute mickey plates would have an empty ear otherwise
