Like a PP, I also use the form letter on the Govt of Canada website. My nieces have also always had their passports with them. Never asked leaving the country. Always asked & been required to show the letter signed by both parents on our return.

We took our daughters boyfriend on vacation with us in August. He is 15 and didn't have a passport but I had his birth certificate and noterized letter. Nobody asked to see the letter at all. I just handed over the birth certificate along with our passports and that was it. I wasn't even asked anything about who he was which I thought was odd given the fact that the rest of of had passports all in the same last name. I figured it was because he was a teenager and not a child.

Just crossed from US to Canada this past weekend without my husband. Birth certificates and a notarized note are all you need. We had no issues.
