Just lost my mom and looking for a certain poem

I am trying to find a memorial poem for a prayer card. I can only remember bits and pieces of it but it was very special to me and I have searched everywhere for it. The parts I remember: don't be sad for me, I've known the love of a good husband, wonderful children, the sound of laughing grandchildren, friends, etc. If anyone knows this poem, I would be so grateful.
