Is this ap voucher valid?

My dd has a Costco voucher for the ap/$50 gift card deal. All it is is a little paper card with perforated edges, about the size of a credit card. All of the info is on it. Is that all she needs to take to the ticket booth? She doesn't have anything else. I can't believe I'm asking this, since I bought 2 of them myself in October, but I just don't remember! But I'd hate to get there on Saturday and find out it's no good because something's missing.

That's what mine looked like (paper, about credit card sized), and I didn't have any trouble using it to get my AP, even though it wasn't very "official" looking
