Immigration forms?

Hi I was wondering what we need to do about filling in the forms, My DD(5) has a different surname to me as she has her Dads surname and we are not married?
 
You need a separate green Visa Waiver form for everyone in your party anyway :)

The white customs form you need one per family, so as your DD has a different surname she will need her own.
 
I think you will have to fill in a diffrerent form for her, you can still go thru immigration together tho.
 
who's traveling?
just you and your daughter?
or you, your daughter and your daughter's father?

if it's just you and your daughter, you fill out one customs form - you're the head of the family of the two of your entering the country..

my guess is, even if it's the 3 of you it would still be one form, but i don't know for sure...
but if it's just the two of you, you fill out one customs form....

maybe gooyish is right and its a separate customs form...
it doesn't really matter.....just fill out multiple forms just in case....they don't take very long to fill out...
and then let the customs person decide...

well as goofyish said, you have to fill out the immigration form for each of you (everyone needs their own form, no matter what their family name is), but the customs form is by family...

it seems to me that she's your family, regardless of name....
my sister has a different name than her husband and sons and they only fill out one customs form...

but for the immigration form, you'll fill out one for you and one for your daugther...
 
To be honest, the white customs form is very easy and quick to fill in so you may as well do one just in case.

Make sure you ask for a few spare copies of the green and white forms when you check in just in case you make a mistake. They do carry limted spares on the plane, but these can run out!

If the immigration officer notices the different surname in your DDs passport to the one on the customs form he may be a 'jobs worth' and send you to the back to fill one out.
 
Thanks I guess I will fill in a couple to be on the safe side.

I am a bit worried after the check in clerk at Paris CDG was reluctant to check us on to our flight as he thought the police would stop us because of DD,:confused3 we were at the check in desk for 30minutes while he called somebody to ask if we could fly and this was all because DD had a different surname.
