immigration card question?

This is a dumb question, but when filling out this form do you put the date of arrival at the first island in for the date? Also Port canaveral doesn't fit in spot for city of departure--what do you put? I know these are supposed to be self explanatory, but ....

They are definitly confusing & not self-explainatory. Just squeeze Port Canaveral into the spot & I don't remember what date to put in. Not sure how it asks the question, but at the terminal they will definitly help you out.

The best thing to do is fill out all of the paperwork with what you KNOW. Post-its or paperclips on the pages with questions. The CM's in the terminal have this down pat, so asking them and filling in the few missing blanks goes pretty quickly. And this is better than guessing at the correct answer and being wrong and then possibly having to redo the entire form.

I had my form mostly filled out, had a couple of questions and had signed it. By the time I had my credit card out so the CM could get the number, she had already taken care of it. They are really quick and efficient.
