Yes, as long as you are within the 120 day window for reservations.

Yes, you can make your reservations on the Disneyland website. I think you can book your park days on the app, too, but I haven't tried that way.

And just to note - you have to reserve them before your trip, unless you want to take the chance that the park you want gets fully reserved, or (worst case) both parks book up and you have nowhere to go.

I just completed my park reservations for July on our 4 day park hopper passes. For each day you pick an initial park to start at for the park hopper, and for an all day ticket you just pick the park you want for that day.
