If you were born in Canada, you must provide an original of either:
•a Canadian birth certificate (issued in Canada by the province or territory of your birth), or
•a Certificate of Canadian Citizenship.
If you were born outside of Canada, you must provide one of the four Canadian citizenship documents below:
•Certificate of Canadian citizenship
•Certificate of Naturalization in Canada
•Certificate of Retention of Canadian citizenship
•Certificate of Registration of Birth Abroad

At least ONE document to support your identity and the name to appear in the passport must be submitted with your application form. Further documentation may be required. For a child, only the proof of Canadian citizenship is required.
Documentation to support your identity must be issued by a federal, provincial, municipal authority, must be valid and must include the bearer's name and signature.

•Provincial driver's licence
•Provincial health care card (excluding Alberta and Manitoba)
•Other provincial identification card
•Other federal identification card
•Certificate of Indian Status
•Old Age Security card
•Federal, provincial or municipal employee identification card
