HSA Tax Help

I'm using Turbo Tax to do my taxes. It is asking for the payer name and address on the 2099-SA form. Is that the name of the bank or our name?? I'm guessing the bank, but I don't get the "payer" part. Anyone know?! Help!

You would be the Payee. The bank would be the payor.
