How to transfer an ADR?

I know there are several threads on transferring ADR's but I'm not quite clear on how you actually do it. Can you have Disney change the name/phone number on a ressie or do most people just give the new person the name the reservation is under?

I called the Dining line and gave them the new person's name and phone number. It was very easy!
