How to start DAS?

My husband and I are planning a trip just him and me in June. He had spinal fusion surgery a few years ago and has suffered some long-lasting effects like neuropathy and shooting pains in his neck and lower back. With our trip pending, I was thinking today that maybe he would qualify for DAS. I have no idea how to start this process. I'm not so worried about getting the 2 advance rides as I am just having it for the park. Him standing for long periods is becoming less and less of an option. I know I have to call between 2-30 days out to attempt preapproval for him, but I don't know what that process is, nor the subsequent steps after. How do we find out if he's approved?
