One would be for a significantly disabled individual traveling with a caregiver staying in a separate room but who needs immediate access to their patient in the event of an emergency. These situations happen very rarely and would be handled by the special services team at Disney's reservations office.
The next circumstance would be when 1 or 2 adults travels with more children than can fit in a single room. For example, 1 adult and 4 children (aged 3 and older) at All Star Sports or 2 adults and 3 kids (aged 3 or older) at Caribbean Beach Resort. Add a 3rd adult to the mix and connecting rooms can no longer be guaranteed.
Pretty much, these are the only situations where connecting rooms would be listed as "guaranteed." Even then, sometimes "guaranteed" connecting rooms may not be available due to extraordinary circumstances.
Finally to the OP, Disney is now being very particular about booking multiple rooms on a single reservation. Unless you all reside at the same address and have fewer than 3 adults in the party, each room will be booked under its own reservation. You can have them linked (as a magical gathering if you have 8 or more people altogether) but they will not be booked as a single reservation.
We just booked three rooms for our trip in May 2007, they are three separate reservations, each requiring a separate deposit. We, 4 adults and 1 child live at the same address. When we booked for May 2006 (two rooms) it was under one reservation with one deposit. Our 2007 room reservations are linked but not guaranteed to be connecting. We were told by AAA that Disney changed the way they book rooms for 2007.
