Go to DD and catch the bus there, or taxi.

You can take a bus from POR to Hollywood Studios and then take a boat to The Swan.

I'd say it depends on the time of year and the company involved.

If the weather is nice, possibly sunset, you could take the bus from POR to Hollywood Studios and walk (less than a mile). It's a nice stroll along the waterway and pretty romantic.
