How do I add links to my signature?

If you look at my signature now, you have a link to my mini trip report, and then the name of the report just beneath it. How do I get the words "It was Just Grand!" to actually be the link. I've searched the boards, but can't seem to figure this out.
I appreciate any help in advance!
