HELP with WPASADI application PLEASE

I have filled it out twice and every time I go to submit it, I get an error message. I e-mailed the help address listed on the application and no response. Does anyone know who I can talk to for help???

Two things...

When I filled out my app I left the IM field blank (because I don't IM), and it kicked my app back.

Then I read on one of the threads that when you input your number between 1 and 100,000 don't put a comma in it if it's over 3 digits. It will kick it out. (so instead of 4,389 enter 4389)
