Help with Tag in Signature???

RedLuvFamily's 10 year old little boy had made a tag for me, but since I had to re-register I lost it. Could someone teach me how to make one??? I absolutely loved the one he made for me.

Was it in your signature? if so, search your old accounts name and find it..
I'm not sure if you are talking about a photo or not?
