Help with Self assessment for review!

Not sure if anyone on here can help but I figured it was worth a try. I need to write a self assessment for my yearly review and I am stumped! I am awful at these things! I know I should try to "sell" myself as an employee but still be realistic. I work in healthcare if that helps! Any help would be great! I really hate thses things did I mention that yet??? thanks in advance!
 
If your company or your boss has not expressed specific expectations for your self-evaluation, then ask yourself, "What would I like my boss to recognize in me?"

It can be as easy as detailing, in list format, the projects you've worked on in the last year, anything special you have accomplished, any training you have attended, and any good feedback given to you by customers/co-workers/managers.

Then go on to list a few things as goals for the upcoming year -- training you would like to complete, projects you would like to work on, or just general things you want to do (these things do not need to sound like "weaknesses" just things you want to accomplish or improve upon).

This is what I would suggest too. Start with making lists. What you have done. What you will be doing. New responsibilities taken on. Challenges you may have had and how you resolved them. Cover backwards the last year and looking forward. Any special recognitions you have recieved. If there have been any negatives, how you may have "fixed" the issue. Suggestions for improvement and how you can or have helped.

Did you have any targets/goals set out for you in the past (as an individual or part of a group)? If so, how were those accomplished, or what is being done now if not completed?

Once you write everything down hopefully a logical presentation will be obvious :goodvibes
