Help with a name tag

Hello all! My DIShome is on the WISH boards, but I always stop here for creative inspiration. Y'all are amazing. I have a small dilemma, my DS8 has decided he wants to dress up for Halloween as a Chef Mickey's waiter. Super cute idea. We've made his apron and he's carrying a napkin to wave, but we need help for his name tag. We'd like it to look like an actual Cast Member's tag that I could mount and put a pin on the back. Has anyone made any of these? If so could you post the template or make one for us? His name is Tad and we're in Missouri. Thanks in advance for the help, it will really make the costume.

I'll help recreate the name tag. I do need a description of the tag as I haven't had the opportunity to see the tag (yet). Does it have Chef Mickey with the spoon and fork? Is it oval or rectangular? What is the color? (Even better if you have a picture of a waiter with the name tag--I couldn't locate one when I googled it.)

Telynn, I looked at my pictures of Chef Mickey and it looks as tho Mickey's is the plate fork and spoon..I don't see his name on it tho, the one I have of Goofy, his name tag is oval like with just his name I hope this helps...
