Help w/ letter for donation gift

A little while back there was a discussion on teachers gifts and some suggestions were to make a donation to charity rather than get the teachers, paras etc 'another' candle......(although I usually give Lindor truffles!)

So, how would you address the letter to the teacher? I want to include everyone that you would normally get a gift from us (my kids have several therapists and resource teachers as well)

My son has a class mate who has been hospitalized and the school is collecting donations for the family to help with medical costs, and I thought this would be a really great opportunity to give a little extra. I just need some help with what to write!!
