Help needed remembering who designed

For me ..two years ago, almost 3, one of the talented designers here designed mickey heads for me...they had the year in one ear and my sons name in the other. Toward the bottom on the mickey head was a pirates of the caribbean Mickey Mouse. My boys LOvED them, wore the shirts out! I'd like to make them new ones for our trip next week. If anyone can help, it would be much appreciated. Names are Luca and Jaxon...year. 2014. Thanks.

Love these designs and my Ds is pirate mad and we go for the first time next Feb could I possibly have this design also his name is Lewis and we go in 2015.
