Help! Form for Traveling with other people's children

My son will be taking a friend on an upcoming cruise. I recall that there is a form that we need to have signed to take someone else's child out of the country. Can anyone tell me where to access that form? Thanks in advance.
 
You should receive a document in your cruise documents if you tell your TA that you are traveling with someone elses child they will make sure Disney sends it in your cruise doc's.

If you don't want to wait on the cruise doc's you can type a letter yourself and have the child's parents sign it and get it notorized. That's what we did and the cruise line said either one is acceptable.

Something else you may not have thought of is you should have (not required but a VERY good idea) a medical release in your possession signed by the child's parents and notorized that authorizes you or your husbandf to seek medical attention for their child.

Just a few thoughts. Hope this helps.
 
I agree 100% with the other poster....we have taken our son's friend with us twice and each time I had them write a letter stating we had permission to take him out of the country and how long the trip was, and where we were going, etc. They signed and notarized it. It also gave us permission to seek medical attention for him if the need arised.

Felt much better having the letter as well as the form.

We're taking my son's best friend with us (leaving two weeks from tomorrow, woohoo!!). He vacationed with us last year too. I did a Google search and came up with this site http://www.ilrg.com/forms/auth-tempguardian.html . It covers medical treatment and temporary guardianship. It is required to be notarized. I feel much more comfortable knowing that I will have this when we travel.
