Grandmas final trip to WDW, need shirt d-signs please

So my "Meme" just announced that she would join us on our upcoming trip to Disney when I'm running the Princess Half Marathon for the 1st time. I wanted to do this before I turn 40!! Here's our cast of characters-

Me-Jenn "Ariel" 72
Hubs-"Eric" 71
Dd1-"Angelica" 01
Dd2-"Jasmine" 05
Mom-"Jiminy Cricket" 51
Dad-"Buzz Lightyear" 45
Bro-"Woody" 75
Uncle-"Goofy" 55
Meme-"Snow White" 31

She's had a rough 2011, in the hospital over Christmas, etc...so we are thrilled she wants to come! I had shirts made the last time we all went and was hoping to get someone (not me, I'm the organizer, SO NOT ARTISTIC) to design me a disney head to go over the pocket of our shirts and we want to do jersey style on the back with Disney names above numbers of the years born. Saw this in DHS and loved it!

I have a Property of style that could easily convert to the jersey style you are requesting. These requests will be my first order of business when I get home from work!! (also pocket size DISigns) Thankfully you have fairly easy characters and nothing I have to go out and search for!

Our shirts will be a teal or turquoise so black is great. We want the Disney Character names above the numbers. Thank you sooooo much!

so the 72 should say Ariel above the number? Jiminy above 51? That kind of thing? do you want an image of the character for the pocket DISign?

Mom, Jenn-Me etc. will be nowhere or on front?

Yes, if you can put Ariel above the 72, Eric above the 71 etc...that would be fantastic! Also if you can incorporate something characterish in the number, that would be an added bonus! For the front pocket, if they're all the same I think that would be easiest for everyone...just something Disneyish with 2012 in it. You're so wonderful for doing this! Thanks again!!!
