The trams that go from the parking lot to the park entrance are not wheelchair accessible. If you can get out of the wheelchair, fold it, and place it on the tram, you can use the trams (Tell the parking lot CM that you have a foldable wheelchair and can ride the tram - they save spots close to the tram stops for situations like that).
If you have a handicapped parking permit, you can park in the handicapped parking area (they call it "Medical Parking"). If you park in a marked handicapped parking spot without a permit, you might get a ticket. If you don't have a handicapped parking permit, let the parking lot CMs know that you have a wheelchair and someone who can't ride the tram. They will give you a spot to park where you will be fairly close and not need a permit.
There is no tram from the Medical Parking at any of the parks. Depending on where in the lot you are parked, it may be a bit of a walk to the park entrance. At MK, you will need to walk to the TTC and then take the boat (a bit farther walk) or the monorial (where you need to go up a steep, fairly long ramp to get to the monorail station).
The TTC boat and the monorail are both wheelchair accessible. If you are staying at a WDW resort (not on the monorail line), your easiest option would be the buses. All the buses are wheelchair accessible either with a ramp or a lift.
