I just recently became an independent travel specialist and I work with a host agency. I was just wondering if anyone had a booking form that they use once they go from a quote to a reservation. If you would be willing to share a blank copy of it, I would greatly appreciate it.

Does your host agency not have one? My host agency has a form that goes from quote to reservation as I progress through. Basically just a "next step" to the quote page. I would check with your agency, mine is on my agency net as a webpage, not a document.
 
Reputable host agencies provide those tools to their agents.

If this host has you pay a monthly fee and gives you nothing in return, you should re-think that host relationship.

Nope, I don't pay a fee at all. She might very well have a form, I never asked her. It's very laid back and I've been doing everything on my own pretty much.

We have an Intranet that belongs to our company..within that-we email a computerized quote providing all vacation details complete with hyperlinks and then that can spin off to a CTO (Customer Travel Order) very thorough and efficient. I can't imagine creating an order any other way?
