? for mothers w/ kids in school

We are going to be moving in the near future and I'll have to get DS's paperwork organized. I know when you register for a new school you have to give the new school the blue book for their shot records but I can't remember who has it. Does the school keep it or does it go back to the doc office? LOL If I'm the one that has it I have no idea where I put it. Can I have another one made from DS's medical records?

I think every State must be different. In Idaho you can register for a database and the schools just tap that you dont have to take in their records anymore.
Your doctor should have the records though.
 
I got DD's records from the Health Dept/ Dr office. No big deal.
 
Florida requires a up to date info. I had to have a yellow physical card and a blue immunization card..the Dr. office always fills it out for me, I have never had to get something from the school.
 

In MD, you bring a copy of their shot records. Our drs have a standard form they use, they photocopy.
 
LOL I hate to tell you but you have the almighty blue book! I put mine away for safe keeping 2 years ago and just recently happened upon them! Good luck finding them, but your dr. will fill out med forms for you, just give them enough notice.
 
Our schools actually supplied us with a form that we had to take to the doctors and have it filled out , that's it.
