will be travelling with my 3.5year old and my 15 month old (who is booked as a lap child on the flights)

My 15 month old has hypotonic CP and Im now wondering if buying her a seperate seat might be better???
 
We bought one for our little ones when they could have been lap babies. We did it for 2 reasons. 1. in case of turbulence and 2. for the comfort of child and adults. With their own car seat, they were used to them and were comfortable, and so were we with not having to hold a baby for 2+ hours.
 
You might be ok. If they get along well and have something to do ( game, book, new toy) then could they sit in the same seat for a while.
 
I also have a hypotonic daughter. She did just fine on two trips down to the world, so four two hour flights total. I understand why you would get advice to get her a seat of her own, but my daughter was more comfortable to be held. I gave her a pacifier and she slept in my arms during most of the flights. Have a great trip!
 

I would buy the extra seat. My reason isn't because of her hypotonia - only you can decide if she would be more comfortable in her car seat or if your arms can handle holding/supporting her for the duration of the flight. My reason is that if you hit bad turbulence, you might not be able to hold onto her and keep her safe. I hit bad turbulence once and had things falling on me, including a coffee pot. It hit unexpectedly while the stewardesses were serving drinks. I can't imagine protecting myself for the falling items and keeping a child safe.
