Based on the evidence I process at my agency, the most common causes of fentanyl overdose in my area is either someone injecting heroine and/or cocaine which ended up being laced with fentanyl of most commonly, someone taking knock off opioid pills such as oxycodone that was made with fentanyl in it.
We investigate and try to determine the origin of the drugs but to date, I am pretty sure we have not charged anyone for the death.
I will say that the overdose victims come in all shapes, sizes, ages economic status. Basically any demographic you can think of. Anyone from the homeless person, the teen, the middle aged housewife, the successful business person.
