What are the locaitons it is available at and can you book it online or do you have to call?

We called & booked mamma melrose @ 2pm with the fantasmic show at 5pm

Hollywood & Vine (buffet) - We ate here last trip w/ Fantasmic. The food was ok, nothing to write home about but not terrible either, but our server was an absolute trip. Best server I've ever had, so much so I wrote to Disney to sing her praises

I'm 99.99% sure you have to call.

yep, you have to call
they do have the package listed in the on-line booking system but it doesnt seem to work
make sure when you book at whichever of the three restaurants you want you make it clear to the CM that you want to book the Fantasmic package and not just a regular ADR

Can you get the Fantasmic package if you are on the dining plan?

Yes, you can get the package on the dining plan. Brown Derby will cost you 2 TS credits though.
