i filled application in, then when i filled DW in, it said application already submitted.
Is it one application per family, as i was uneer the impression everyone had to fill one in.

It's one per person so something has gone wrong somewhere. You didn't put her passport number in by mistake instead of yours did you? Maybe a silly question but would she have done her own and forgotten to tell you?
