Can you tell I have been surrounded by this stuff for FAR too long? LMAO. That's what happens when you play EQ and WoW, belonged to a gamers society in college, played DnD weekly, and date someone who does all the above as well.

The most common explanations I have heard come from the RPG Dungeons & Dragons and was in reference to rolling a 1 when a 20 was needed.

We've used it more than just for the Dungeons and Dragons game.... It usually means a failure of epic proportions. A goof-up so big it deserves a tagline like that.

Well, that "All your Base" reference was because of bad translation from the original game the phrase came from....
