EPCOT Passport Examples

Where can I find an example of the handmade/homemade Epcot passports that I keep hearing about? Is there a link to ideas for this somewhere?

I don't even have a copy of the one you can get in the parks to use as an example if I wanted to make my own.

Can someone please point me in the right direction? I would like to make our own for our trip.
