ECV Rental - Front of Scooter Sign Template?

I recall someone posting a template they used for a personalized sign they made for the front of their rented ECV scooter. My mom is coming with us and is handicap and will need a scooter (rented from D and R since they are the vendor for CCI). Was hoping to surprise her with a personalized name sign when we get it and recall someone made a template. If that is you, or you know how I can find it, please let me know. She is coming with us to the Halloween Party this Friday and we are all excited.

That may have been me that mentioned the license plate. This was designed for my mom by Dee over on the Creative DISigns forum http://www.disboards.com/forums/creative-disigns.105/ You can look through the threads. There are a few designers that create these.
