I don't think Hawaii-->Vancouver-->Hawaii would be a problem, since that combination would start and end in the same US port. My wife and I did the Vancouver-->Hawaii-->Vancouver B2B this past year with no problem.
I'm trying to remember the cruise that was not allowed. It may have been the SD -> Vancouver-> HI leg that someone on the boards wasn't allowed to do.

Yes, that would not be permitted under the PVSA rules, since it would be a cruise from SD to Honolulu, two US ports. You could book the add the return to Vancouver: SD -> Vancouver -> Honolulu -> Vancouver as a b2b2b, but likely had to talk to a supervisor in order to do it.
 
I tried to do the San Diego to Vancouver to Hawaii to Vancouver.. B2B2B. Which I think actually meets the legal requirements ending in a foreign port but I think DCL was/is snakebit and wouldn’t allow me to do this so I dropped the San to Van part.

A totally legal routing, probably flagged simply because of the fact you had booked the first and second leg. They would not budge or you didn't try ?

my guess is that they were afraid I'd not take the 3rd leg (illness, miss the ship, etc) which would technically cause me to get on the ship in San Diego and get off in Honolulu.

That sounds plausible. If you did cancel the last leg of the trip after booking the first 2 legs, they would be in violation of the PVSA when you disembarked in Hawaii. Rather than come up with a system for checking if people who cancel cruises might then cause a PVSA violation, they decided not to allow any combination of trips that could result in a violation.

A trip that leaves and returns to the same US port must also visit a foreign port.
A trip that leaves one US port and ends at another US port must also visit a distant foreign port.

I am guessing cruise lines can get away with going from, say, Port Canaveral to Key West and then going elsewhere in the Caribbean before returning to Port Canaveral because they can ensure all passengers get back on the ship, which then stops at other non-US destinations.
