I've almost always had to wait in line and have it dispensed. On really busy days they might have a few waiting off to the side for a CM to hand you. Rancho del Zocalo has a tap and small plastic cups instead of giving you a cup for ice water. Troubadour Tavern also gives you a cup for the drinking fountain instead of dispensing. I've never had a problem anywhere else I've asked. Tomorrowland Terrace and Mint Julep Bar also have small taps where you can refill your own bottle. The only one I can think of in DCA is near the bathrooms at Pacific Wharf.
They definitely don't mind giving away a small water at any soda fountain but honestly the dland water is not very tasty at all. The first day we made it through.. the rest of the week we brought in bottles to get away from the weird water after taste.

Yes, we never had any issue getting free water. It was a little weird tasting though. It was okay when it was really cold but as it melted you could taste it more.
I asked for just ice a few times and just ate the ice to stay hydrated.
You can ask, but they will still often give you a small cup. I always requested a large cup of ice water, but more than half of the time they still put it in the kids-sized cup! It might depend on the location and/or the CM.
