Does anyone have a ADR planning chart?

I hope you understand what I mean...
We are planning all of our ADR's for our trip in January 1st.
Does anyone have a chart that you can fill in with dining plans and later your confirmation numbers? I thought it would be easier if I had a chart with days of the week, with parks tentative ee schedule on there, so I can map out where we want to eat.
There are 12 of us, so it takes a little more planning to be able to eat our meals together.

I just made my own chart using excel. My friend prints out the calendar pages out of Outlook to use. I made a chart in excel that has the days of the week. Each block is broken in to 3 sections am, noon and pm. Then I just typed in the info that I wanted-what park, meal place, time, ressie#. I also made a small chart that I am going to laminate to carry with me that has just the date, resturant, ressie # and time. I hope that helps you some. Someone out there might have a like to a pre-done chart.

Okay....don't laugh, but I tried to do the same and it didn't work for me...do you have yours saved you could email to me??
