Documentation needed to take niece on cruise

We are taking my niece (15) on a cruise in October. What type of documentation will we need. I know I need a notarized statement from her parents allowing me to have her treated in a medical emergency. Do I also need a notarized statement allowing me to take her out of the country? What other documentation will we need.
 
I believe you need a notarized letter allowing you to leave the country signed by both parents. Many have said they did not have this, but we have one for a friend of ours that will be traveling with us. Also, remember to get a copy of the insurance card-JIC
 
There will be a card in her cruise docs that you have her parents sign and notarize, When you check in DCL will keep the copy that is with the docs. Not a bad idea to get a notarized letter also for your own records. Don't forget her birth certificate.

ETA: At least that is the way it was when we went. You might want to call DCL to be sure.

Great suggestion about taking the insurance card. We have taken oldest GS on 2 cruises with us. Both times we had Dad and Mom sign & notarized although dad has custody. We didn't want any problems so we covered all the bases before we got there. One less thing to worry about. :)
