I've seen worse:

Years ago a neighbor of mine came over, barefoot and carrying her baby. Baby dropped its pacifier, Barefoot Mom picked up the pacifier by the rubber part with her toes, transferred from toes to fingers, and popped it right back in Baby's mouth.

So, at least the mom in the OP's story "sanitized the pacifier with spit" and never put her toes on it at all.

Nope, never did it. Maybe that's why my old co worker used to stick her contact lens in her mouth...instant sanitizer.
