We had our table decorated with Mickey confetti at California Grill..I was hoping for a free dessert, but we didn't get one Animal Kingdom was by far the best place to wear our pin - the castmembers there went truly out of their way to Congratulate us...even if we were more than 10 feet away and they noticed the pin they yelled Congratulations til we turned around! At the rainforest cafe` we were treated to three servers singing to us while they presented us with a complimentary ice cream sundae. I loved wearing the pin - just added to the magic!
When I was there for my B-Day they sent ballons and a signed card from Mickey that was waiting in the room for me on my birthday... We went to MVMCP that night and the CM at Pirates sang happy birthday while we were walking in (terribly off key but it was fun).
This year we are going to CRT on the night of my birhday and MVMCP the night before so I will be able to start wearing my pin exactly at midnight...It't the one place that I love celebrating a birthday!

Our son celebrated his 2nd birthday on our last trip and servers noticed him wearing his "It's my birthday" button at every meal. He was showered with birthday wishes and free desserts. In addition, at his WCC birthday dinner, our server recruited the entire restaurant (full by this time) to sing Happy Birthday to him. He is 4 now and still remembers it.

We did this for my Sis IL when we stayed at AKL. It was a little pricey but we wanted to make sure her birthday was not lost in the excitement of a trip. We had it delivered to the room.
I think that celebrating at Disney is fun and for the most part the CM's enjoy making these trips extra special. My DH was "tormented" all week because my DGD insisted on telling the servers that it was her "Pa's birthday. They seemed to get a kick out of watching a 5 YO tease this big guy.
Now for my DGD's 6th birthday we took her to WDW to celebrate and again she was given "special" treatment. We were on the DDP again so her ice cream treat was included in the meal, but at time was decorated.
I would say that if you go expecting special treatment all week long you will likely be disappointed at times, but if you go with the intention of enjoying your special trip and any well wishes are a bonus you will have a wonderful experience.
We did this last year for my dad's bday. I had it noted on the reservation for Pop and on our ADR for Boma's. When we checked in he received his bday button. He didn't want to wear it but we made him and he received bday wishes all week at the parks. Then for our ADR they also had made him up a button (oops..didn't need another one) and we had ordered the small cake. That was the best cake and was good for days afterwards. The waitress almost forgot the cake we had ordered...i guess she missed it on the ADR but when i caught her away from my dad she went and got it. Unfortunately we were all too full from a terrific meal so she boxed it up for us to go. We sure enjoyed it later in the room. We also received a surprise of balloons and a signed card from Goofy and friends the day of his bday. My nephew sure enjoyed those balloons cause Papa (my dad) let him play with them. After the trip he said he enjoyed it all but as it was going on he was a little embarrassed by all the attention. it was a great surprise for him as he didn't expect anything.
