Just wondering if I need to call Disney dining to get this set up or the restaurant directly?

Does this cost anything?

How far in advance do you set this up?

You will need to call Coral Reef directly. They are great to deal with. There is no charge. I booked it one week out last August for my DH 40th birthday. Good luck.

i just did it and the personalized menu (as documented in my trip report with photos).

you should do it as soon as you can (assuming you have an ADR) as they limit the number of signs per night.
