Definately call Chase and let them know you will be charging a large sum on your card. Chase even has a credit card for medical issues. I opened one up for my Dentist. Seriously, who has $4,000 laying around, or $10,000. Prayers for your medical situation.

My car dealership will only allow a $3,000 charge.

My surgeon and hospital gave me a 10% cash discount for paying my bill off with my Disney card (we have a 6,000 deductible). Call and ask what type of discount they will offer. A friend of ours with no insurance was offered a 25% discount at the hospital by paying in full on her visa card.

Most hospitals/surgeons will also set up a monthly payment schedule with low or no interest rate. If you cannot pay the full balance at once, ask for monthly payments and use the card to make the payments. If you are carrying a balance on the disney card, it counteracts the points by paying interest.
