Disney Stock - Certificate of Foreign Status ...

I received a Disney stock last Christmas as a gift. This is the second time (I think) that I've received a Substitute W-8BEN form (Beneficial Owner for US Tax With-holding). The form I got this past week seems similar to one I filled out last January.

For those who've had their Disney Stock longer than I ... is this normal? Will I need to do this every year? And, short of signing that I am a resident of Canada ... is there anything else needed for this form? There is a section that makes mention of Tax Treaty Benefits (if applicable).
 
If it was purchased through a seperate brokerage, you have to submit a new W-8BEN.

If its through the same brokerage, it just may be a matter of process to send out the document.

I don't have Disney stock, but i do have stock with my employer, and have to resubmit my W-8BEN every few years to my brokerage.
 
Mine had someone elses name on the inside. Very weird. I got it as a Wedding present. it costs me more to cash the cheques so I dont bother. i just want the framed stock.

The guy on the phone said given the fact I own one share it isn't a big deal as it won't make a difference on US income since they have a certain threshold you have to meet before you have to pay taxes.
