Disney Shareholders - received dividend statement?

For anyone who is a Disney shareholder: Have you received your statement regarding dividends received in 2004 (form 1099)? I am close to having everything I need to file 2004 tax return, but I have not received anything from Disney regarding dividends I earned in '04. I received the annual report and proxy statement, nothing else.
