Disney Movie Club Survey on Disney Plus

I received an email from Disney Movie Club asking about signing up for Disney plus. Questions were if you were aware of what it is, select top three types of programs you would be interested in, did you sign up for multi-year package.

I signed up and said I would receive a code and never did.
