Disney Castaway 5k?

They do ask that you register prior at guest services and I was given a ticket, though in disney (not rundisney though) fashion, they will let you sign up that morning. Depending on the ship and itinerary, numbers vary. The Eastern Carribbean doesn't arrive until later in the morning, so numbers are reduced. Less people on the Classics mean less runners. I think there were maybe 100 - 150 last March on my Wonder cruise. Hoping for the same on the Fantasy in two weeks.

I was on the classic, and registered the day I boarded but that morning they just had a lot of people show up I guess - Fathoms was completely packed.

Yes, I know they hand out tickets at the Port Adventures desk. A big problem, IMO, is that it is printed in the Navigator everyone gets the night before CC telling you where and what time to just "show up". So anyone who had no clue about the race is now showing up without being pre-registered causing a delay in getting everyone of the ship in a reasonable time.

I'm on the same cruise as you and it's been sold out (except for concierge) for a very long time so I'm anticipating a big turn out. Meet time should be 8am in the Walt Disney Theater. All ashore time for guests is 8:30. There is no way they can process hundreds of people in order to get us off the ship before other guests.

Does anyone know if the nursery is open? My friends traveling with us will have a one year old, it would be nice if both parents could run.

Yes.

But notice the previous post(s) about getting off the ship late.

I personally have never been among the first off when doing the normal free island 5K. One of them took so long that a mom was livid because she basically had run out of her one hour nursery time just after getting off the ship. So book it for 2 hours or even more if you can (not sure).

I've never dealt with unregistered people (since they went with pre-registration), but it's never been a well-oiled machine.
