Dinning Reservation Card (like DoNatalie's) *A few more Please*

I was wondering if someone could make me a Dinning Reservation Card like the ones Natalie has in her Photo Share Album. I have used her cards for all of our Disney Vacations and they work great. However, she does not have one for Be Our Guest or Tony's Town Square and I need those to complete my book.

Since she is no longer designing I was hoping someone could great those for me.

UPDATE: I need a few more cards to complete my planner for our upcoming trip. Please see list in Post #8. If anyone can create these I would really appreciate it. Thank you so much for taking the time to look at my request. I really appreciate all the Magic you share.
