Dining Plan Gratuity question

We had deluxe dining last time (January 2009) and we had breakfast at CRT. When the bill came, the waiter stated that gratuity was included and no need to leave an additional amount and there were a select other restuarants in WDW that gratuity was included (and I can't remember which ones he said).

Do you know which others? We are going back in June and on the deluxe dining again with plans to eat at CRT again. I thought maybe HDR- which we have ADRs for as well.

The gratuity is included at CRT, for room service/pizza delivery, and at dinner shows - Hoop De Doo, Mickey's BBQ, and Spirit of Aloha.

Disney Dining Plan brochure notes that gratuity is included for CRT as part of the two credit requirement.
