Different Charity Projects - Help

I've got a thread on the CB asking for pet photos for one project:

I've also got a few more in the works, but figured I'd ask here. Does anyone have a cookbook template, or a suggestion of where I can download one?

Also, where's the best place to get cardstock paper to print out the projects, and what is the deal with all of the weights? I know I want the cardstock to be a decent thickness, but I have no idea what all the weight stuff means.

Any help would be GREATLY appreciated.
