DDP Question about add-ons

I've been checking out a lot of menus for our ADRs this coming week and I've noticed a few entrees that have add-ons - like, add chicken or shrimp for $2. Sometimes you can add a cup of soup to an entree for extra.

How does that work with the DDP? Do you have to pay OOP for the add-on? Seems like it should be included, especially if the total dollar amount is still less than the most expensive entree on the menu.

Seems silly if I can order Prime Rib and have it all covered, while DW orders something much cheeper like pasta yet have to pay extra to add shrimp.

Also, is the Pirate Punch with the clip-on cup topper included on the DDP?

I think that add ons are not included and must be purchased oop.I know that they allowed me to substitute an item but I had to pay extra for add ons.

I know that for a Cesar salad they were covered - I asked last week but did not end up ordering it. I also know that the friend we traveled with did the menu listed shrimp for chicken substitution in a pasta at Mama Melrose last week and that was also permitted.
The plan did not cover extra/ala-carte side dishes.
The drinks with the clip-on cup topper were also not covered.

OP Can you give us two specific examples? Restaurant, meal and menu items?

The ony upcharges I recall we for kids meals for something like steak at some TS places.

For example:
At Kona Cafe Pan Asian Noodles are $1 more if you want shrimp in it.

At Wolfgang Puck Express you can add roasted Julienne chicken to any pasta dish for $2

At Le Cellier you can add smoked bacon or sauteed mushrooms for $1 each to any of their sandwiches
